Prepare for Your Care grew out of a desire to help make medical decision making easier for people and caregivers. It is the result of ongoing research, collaboration, and generous funding.

History

2008

Created and tested an easy-to-read advance directive proven to be effective in a randomized trial. It is available in 10 languages. See the Research »

2009 - 2012

Conducted studies with diverse populations to further understand patients’ and caregivers’ needs for advance care planning. See the Research »

2013

Launched the PREPARE website with video stories that help people engage in advance care planning.

2014

Conducted a pilot study in senior centers showing PREPARE increased engagement in advance care planning from 40% to 100%. See the Research »

Launched Spanish version of PREPARE.

2016

Funded by the Gordon and Betty Moore Foundation to update the easy-to-read advance directives for all 50 states. Forms will be available (in English and Spanish) in 2017.

2017

Visited site: 95,000 unique visitors from over 150 countries.

Rebecca Sudore, MD, FAAHPM, Professor of Medicine, Geriatrics Division, University of California, San Francisco.

Dr. Sudore is a geriatrician, palliative medicine physician, and advance care planning researcher. She also directs the Innovation & Implementation Center in Aging & Palliative Care (I-CAP) and the Vulnerable Populations for Aging Research Core of the NIA-funded Pepper Center.

Dr. Sudore is passionate about making medical information easier to understand and helping people make informed medical decisions.
