Our PREPARE team stands with many other organizations in condemning the long-standing and ongoing oppression of Black people and communities of color. In addition, our team's mission is to continue to fight against racial inequities in health care access and advance care planning (ACP).
Our team has gone to great lengths to include diverse patients, caregivers, and communities in the design, development, testing and implementation of our PREPARE materials.
We co-created the PREPARE online program, our easy to read advance directives for all 50 states in English and Spanish, and our California advance directives in 11 languages with many individuals from diverse cultural and family backgrounds. The PREPARE online video stories are also based on real-life stories from diverse individuals who participated in our focus groups and in-depth interviews.
The COVID-19 pandemic has hit communities of color the hardest. We have created easy to read materials in English and Spanish to help diverse communities consider their options for medical care and plan for a hospital visit. We have also provided easy to read scripts that any type of clinician or lay health worker can use to start an ACP conversation with their patients or clients.
We are also pleased to share resources from other advance care planning advocates and organizations:
