The PREPARE website is copyrighted and its use is subject to the License terms listed below.
PREPARE can be used freely for personal use.
PREPARE cannot be used for profit. It also cannot be used by for profit or non-profit healthcare organizations or other for profit or non-profit organizations or agencies without express permission from the author: Rebecca Sudore, MD (email@example.com) and the University of California.
For instance, PREPARE cannot be used within a defined clinical program (such as an advance care planning program) or for research purposes, including studies, grants, quality improvement projects, or any form of publication without express permission.
The PREPARE link can be added to a list of resources. However, a licensing agreement with UCSF is needed if the PREPARE website is used as part of a clinical program, such as a defined advance care planning initiative, or as part of a research or quality improvement initiative.

OWNERSHIP: All right, title, and interest in and to the website known as PREPARE (“Licensed Material”) shall at all times remain with The Regents of the University of California (“The Regents”). User acknowledges that the Licensed Material is a copyrighted work and as such is protected by the copyright laws of the United States and by international treaties. User shall not copy, distribute, modify, or publicly display, nor permit any of its personnel to copy, distribute, modify, or publicly display the same for any purpose that is not specifically authorized under this Agreement.
PERSONAL OR CLINICAL USE: If the Licensed Material (PREPARE) is used for personal or non-commercial use, it can be used without the right of a sublicense. In this case, the UC Regents grants the User of Licensed Material a fully paid-up (i.e., no fees), nonexclusive, nontransferable, revocable right to reproduce, distribute, and display publicly.
If a healthcare agency or organization or non-profit organization would like to use the Licensed Material on more than a one-on-one or individual level (for instance it would be used on an organization/agency website or intranet), then the Users must contact the Material’s author, Rebecca Sudore, MD (firstname.lastname@example.org) to discuss intended use.
All Users further agree that they will not provide the Licensed Material to any other individual or organization not specifically authorized by this Agreement. The user cannot modify or change the Licensed Material without contacting the Material’s author, Rebecca Sudore, M.D. (email@example.com) to discuss the intended use and modifications.
RESEARCH USE: If User plans to use Licensed Material for any and all research purposes including, but not limited to a research project or study, grant, grant application, or quality improvement project with the intent of publication, the User must first contact the Licensed Material’s author, Rebecca Sudore, M.D. (firstname.lastname@example.org) to discuss the intended use. Also, the user cannot modify or change the Licensed Material without contacting the Licensed Material’s author.
INDEMNIFICATION and LIABILITY: The User agrees to indemnify, defend and hold harmless The Regents, its officers, agents, and employees from and against any and all claims, liabilities, demands, damages, losses, costs and expenses (including costs and reasonable attorneys’ fees) or claims for injury or damages that are caused by or result from User’s use of the Licensed Material, including but not limited to, any use of the Licensed Material that is not authorized under this Agreement
USE OF NAME: Nothing in this Agreement shall be construed as conferring rights to use in advertising, publicity or otherwise any trademark, trade name, service mark or the name of "University of California" or "The Regents of the University of California" or any abbreviation thereof. However, the use of The Regents' name as associated with the copyright notice for the Licensed Material shall not be restricted.
User understands that THE REGENTS MAKES NO REPRESENTATIONS AND EXTENDS NO WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED. THERE ARE NO EXPRESS OR IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE, OR THAT THE USE OF THE AUTHORED WORK WILL NOT INFRINGE ANY PATENT, COPYRIGHT, TRADEMARK, OR OTHER RIGHTS.
